Investigation of a hydrogen compressor explosion
An explosion and fire in a hydrogen compressor station within a large oil refinery was the result of substandard conditions and failure to systematically address all significant problems on a critical facility. Major modifications and a formal process safety management program have enabled the facility to expand and achieve a high level of safety and reliability. The willingness to share this information is an indication of the organization's commitment to process safety excellence within the industry as a whole. Industrial accident databases help to promote such information sharing and ultimately lead to more effective solutions to common industry safety problems.
