Musculoskeletal tissue regeneration renders significant functional restoration and increased quality of life. Conventionally autogenous bone graft demonstrated reliable bone fusion in fracture and spinal instability despite of morbidity related to harvesting procedure. Recently introduction of multipotent mesenchymal stem cells (MSC) provide useful tools for regenerating bone, cartilage, muscle, and ligament. In addition to cell therapy, genetic modification using anabolic gene(s), biocompatible scaffold, and biophysical stimulation i.e., cell culture under repetitive stretching also facilitate expression of desired phenotypical expression of certain tissue. In this lecture, firstly, intervertebral disc regeneration using anabolic gene therapy, scaffold, and MSC would be introduced. Secondly, spinal fusion using gene therapy, MSC, bioglass and ligamentum flavum would be discussed. Thirdly, interim result tissue engineering using MSC, polyurethane, and bioreactor with stretch and torsional strain would be presented. Even though tons of data regarding tissue regeneration and/or engineering had been published, still there is unsolved problem in that matter. Therefore, multidiscplinary approach to derive a strategy to overcome problems regarding tissue regeneration is desperately needed. |
